Donate Through the Mail or in Person

Please print the form, fill it out and send it to the address listed on the form. Don’t forget to mark Programs and Services for Older Persons in the “Apply my gift to” section and list the program you would like your donation to support.

Be sure to select the PSOP program of your choice in the “Designation” drop-down menu. If your desired program is not listed, select “Other” and write in the PSOP program of your choice.
